After speaking with your claims professional, you will understand your type of injury and the next steps for a treatment plan.

Type of injury	Minor Injury Guideline	Non-catastrophic	Catastrophic
Description	Soft tissue injuries including whiplash, sprains, strains, cuts, bruises or partial muscle tear. See the full Minor Injury Guideline.	Injuries include broken bones or full muscle tears	Injuries include severe brain injury, blindness, amputation, or paraplegia. See the full definition in section 3.1 of the SABS.
Form to Complete	Complete and sign the Accident Benefits Application (OCF-1).
Treatment plan	Your treatment provider will prepare a treatment confirmation form for you to sign. Treatment and resolution with medical and rehabilitation benefits are limited to $3,500.	Your treatment provider will prepare a treatment plan for you to review and sign. For catastrophic injuries, you will also need to complete additional forms.
